Regrettably, the Primary Care Initiative (PCI) will close its doors as of December 20, 2011. Greenwich House will continue to provide primary care services at other programs that already offer them and will ensure that every patient has their care transferred to another provider before December 20.
We have worked with our patients and community partners to identify new providers of choice in the network of managed care plans. Medical records are being sent ahead of time to new providers, to ensure uninterrupted medical care.
Greenwich House will ensure availability of all patient records. The Medical Record custodian is Janet Walpole, Corporate Compliance Officer, who can be reached at 212-991-0003, extension 248 (phone); 646-365-5730 (fax). Medical records will be kept for a period of at least seven years.
In addition, a Greenwich House staff person will be available for at least three months after closure date to respond to telephone calls to the PCI clinic voicemail.
