Case Management

Case Management is headquartered at Center on the Square at 20 Washington Square North and works to assist an ever-increasing number of seniors with achieving the highest quality of life and obtaining needed resources available specifically for seniors in order to enhance their daily existence. The program helps older individuals apply for or re-certify benefits, entitlements and other services available to them.

Who Provides Care?

The Case Management team consists of:

  • A part-time intake caseworker,
  • A full-time master's level social worker
  • A full-time social work supervisor
  • A graduate-level social work student intern                   

Services Provided


  • Assist with questions about SCRIE
  • Provide services to Spanish speaking seniors
  • Maintain an extensive file of resources and information on legal aid, meals-on-wheels, affordable dental care, mental health providers and a host of other services
  • Provide referrals to Money Management or Senior Health and Consultation services, where necessary

Case Management staff regularly visits the other centers to provide services.

Case Management receives new clients primarily through referrals, from places such as 311, other senior centers, medical providers, family, landlords, DFTA, Village Care, friends and neighbors. The needs of many of these referrals are complex, reflecting the fact that several of the seniors suffer from mental illness or dementia and have very limited or fragile support systems.
